One form is used for SIDA, Sterile Area, ID badges and some types of AOA badges. Employees complete Section I of the badging form. The company Signatory Agent selects the badge type in Section I and completes Section II. This form must be turned in to the badging office before the employee is scheduled for class. Signatory Agents or their designated representatives can obtain copies of this form from this web page or the Badging Office. Once obtained, they can also be reproduced locally.  Click here for the Badge Application form.

Pay special attention to Section II of the form as it includes questions for key issue, driving status and actions required by the Signatory Agent.

Check yes under “Ramp/Jetway Doors” if you wish an Intellikey issued to your employee to open Ramp and Jetway doors. If not, check no.

Check yes under “SIDA/AOA Driver Permit Required” if the employee's job requires him/her to drive on the ramp. If not, check no.

A signature in Section II certifies the following:

Company's responsibility to notify the Airport Badging Office, Airport Police or Control Center immediately if an employee is terminated or loses his or her badge.

Company's responsibility to reimburse the Indianapolis International Airport for any TSA fines levied against the airport that is caused by an employee's failure to adhere to the Airport Security Program.
